We have just entered 2026, and we are already celebrating milestones.
With the opening of two new PACE centers, the vision we unveiled last May at On Lok Celebrates, as part of our Building Tomorrow, Together campaign, has become a reality. In less than 12 months, with you support and generosity, we transformed blueprints for buildings into real places of care, safety and community. What a way to start our 55th anniversary year!
Together, we are investing in a future where aging is supported, celebrated, and centered on people. Our two new locations in Union City and South San Jose bring critical services closer to home for hundreds more seniors and families in the South Bay—where On Lok is the only PACE organization serving all of Santa Clara County—and into East Bay neighborhoods where our vans are already becoming a familiar sight.
Both centers reflect, in their design, amenities, and spirit, everything we have learned and believe about aging with dignity, independence, and human connection. Current and new On Lok PACE participants in East and South Bay have the option to receive services at these new locations. We already see them becoming lively hubs of connection, with plenty of room to serve more.
Our growth is not reflected just in capital investments: The trust we have built within the community since we launched the first-ever PACE center remains the greatest measure of our success. We have invested deeply in people, partnerships, and community relationships to break down barriers that too often prevent access to care: lack of continuity and coordination, fragmented systems, and mistrust of providers.
Last year, we delivered on the core goals of our three-year plan. We expanded Club 75 in San Francisco to better support LGBTQ+ seniors with culturally appropriate programming. Today, Club 75 is an alternative care setting where On Lok PACE participants can receive activity therapy with their LGBTQ+ peers. Our groundbreaking HIV caregiver training partnership between On Lok Nursing and UCSF is now in its second year, helping to transform care for LGBTQ+ elders.
We continue our strong partnership with the San Francisco Department of Disability and Aging Services (DAS) and we are growing our collaboration with San Francisco Health Plan to support their most vulnerable Medi-Cal members through On Lok Enhanced Care Management and On Lok Medically Tailored Meals. With increased state investment in community-based care through the CalAIM initiative, we are confident we can expand these life-enhancing programs to other counties and more people.
We are a source of information for our communities, monitoring state and federal changes in funding and policy that could negatively impact Medi-Cal beneficiaries. And, as the founders and innovators of the PACE model, we never stop championing the quality and integrity of PACE, especially as more providers, including for-profit organizations, enter an increasingly competitive healthcare marketplace
