On Lok Medically Tailored Meals (MTM) are provided to SFHP Medi-Cal members at no cost to support their recovery and manage their health condition.

Medically tailored meals are designed by a registered dietitian based on the primary care physician's recommendations.
Meals are home-delivered ready to eat, Monday through Friday.
The On Lok Medically Tailored Meals benefit includes up to four sessions with a registered dietitian to learn how a nutritious diet can help manage health conditions and improve quality of life.
