Participants at On Lok PACE San Jose have been showing off new hats and scarves in an array of colors and styles. Created by Center Director Annie Zacanti, the hats and scarves reflect the special relationship our staff have with our seniors.

“My Noni taught me to knit as a child, and now I knit for relaxation,” said Annie. “I enjoyed making the hats and scarves, and seeing our participants wear them in the center brings me a lot of joy.”

Three participants were happy to share photos of their new hats and scarf as well as their experience with On Lok PACE.

Xushan Wu and her husband heard about On Lok PACE in 2017, through another couple who loved the program. To this day, Ms. Wu is very happy they decided to try it. At the On Lok PACE San Jose Center, they have found support for their health and wellbeing and daily activities that bring joy and connection. The comprehensive care they receive has been a tremendous help in every aspect of their lives. Today, their entire family shares deep gratitude for the support On Lok provides.

After Grant Piatt joined On Lok PACE in 2024, the San Jose center quickly became a place that felt like home. One of his favorite activities is bingo, and he rarely misses a chance to play. But what matters most to him is the kindness he encounters every day. Mr. Piatt says he deeply appreciates the medical care, the staff, and the friendships he has found at the center. Spending time with other participants has become an important and meaningful part of his day.

At 93 years old, Lanyin Yang Ho says On Lok PACE has become an essential part of her life. She speaks warmly of the doctors who carefully manage her health and her medications, and the friends she has made at the center. And there is one thing she never misses: the meals. Ms. Yang Ho laughs as she explains she prefers eating at On Lok because the food tastes wonderful and is prepared with exactly the right texture for her.

For these seniors, On Lok PACE is more than a place to receive the care they need—it is a community that truly feels like family.
