Prevention is an Important Part of Your PACE Benefits

A message from Dr. Jay Luxenberg, Chief Medical Officer, On Lok Lifeways

When you enroll in the On Lok Lifeways Program of All-inclusive Care for the Elderly (PACE), you are assigned a primary care physician who is part of an interdisciplinary team skilled in treating aging adults. The interdisciplinary team also includes PACE center nurse practitioners; home care nurses; a social worker; physical, occupational, speech and recreational therapists; and other care professionals and support staff.

The team develops a personal care plan that addresses senior health, brain function, social stimulation and general quality of life. Every aspect of the plan is designed to keep you or your loved one living independently in the community for as long as possible. Your physician meets regularly with other members of the team to review your health and wellness and make adjustments to your care plan as your needs change.

Your doctor and nurse practitioner work closely to coordinate and schedule all needed preventive, primary, acute and long-term care services, including annual checkups and screenings. In addition to visiting a physician when an illness or injury occurs, on Lok Lifeways participants have regular checkups to monitor diagnostic readings (heart rate, blood pressure, etc.) and, if necessary, receive prescriptions for medication and/or treatment to bring these readings to healthy levels. Routine tests and screenings, check for cancer, diabetes, heart disease and other conditions. These tests, and other preventive services, are also part of the Medicare Annual Wellness Visit.

Seniors receive most of their care from clinical staff in their PACE center. During the year, however, your physician and nurse practitioner may schedule preventive visits with medical specialists to promote your longevity and optimize your health and function. Specialist visits may cover:

Dentistry and dentures

Optometry and eyeglasses

Audiology and hearing aids

Podiatry

We have contracts with many specialists and other providers in the community to make sure that you receive the care you need.These visits must be authorized by your care team, and payment will be covered by PACE.
