On Lok Lifeways may be right for you if you or your loved one:

} Is 55 years of age or older

} Lives in San Francisco, Fremont, Newark, Union City or 
  Santa Clara County (not including Gilroy, Morgan Hill or
  San Martin)

} Requires ongoing medical care and supportive services to 
  continue living at home

} Is able to live in the community without jeopardizing
  personal health and safety or that of others
