On Lok Lifeways may be your perfect alternative to a nursing home.

Our integrated teams of elder care specialists work closely with seniors and their families to develop programs that keep seniors living at home for as long as possible. We value participants' independence and self-determination. Many of our professionals are bilingual, and we honor participants' diverse heritages.

On Lok Lifeways may be the perfect senior care plan for the person who:

• lives in San Francisco, Fremont, Newark, Union City, San Jose or certain other cities in Santa Clara County, California;
• lives with chronic illness or disability that requires ongoing medical care and support services, equivalent to skilled nursing home care
• can live safely in the community, without risking the health and safety of themselves or other people; and
• is age 55 or over.

Affordable Senior Care

On Lok Lifeways accepts Medicare, Medi-Cal and private payment. All eligible seniors can participate in On Lok Lifeways even if they do not receive Medicare or Medi-Cal benefits.

Most On Lok Lifeways participants have no out-of-pocket costs, while others have defined monthly payments.

The Medicare/Medi-Cal benefits you receive will determine your individual share of cost for On Lok Lifeways.
