- One of the top banks in California, Pacific Bank has commissioned e21 to implement its Web Suite product as a based foundation for their Securities Portal initiatives.

April 2003
Hisense USA - A manufacturer of consumer electronics, Hisense USA challenged e21 to create a complex website where they can sell their products online, market to potential and existing customers, and support customers who purchased products from them.  e21 delivered this solution and the site is now active.

Web Suite Overview

e21's Web Suite product is a comprehensive and affordable solution for small to midsize companies who want to provide information about their company via simple content management, sell their products online, execute and managed email marketing campaigns, provide customer service and product support, as well as solicit customer feedback using online surveys on an integrated manner.

The Web Suite product accomplishes this enormous task by providng companies with several key applications.

Content Management - A simple to use application that enables any user to create and update website contents and information without the need to know programming. The solution contains a user-friendly WYSIWYG editor very similar to Microsoft Word for creating, modifying, and formatting web pages. This application also allows for managing different types of contents such as hyperlinks, frequently asked questions, and downloadable files.

Product Management - A comprehensive application for creating and managing detailed information about products companies are trying to sell online. This is a generic solution where users can define product specifications, descriptions, warranty information, images, packing details, categories, etc. The strength of this application is its configurability so regardless of what tangible product you have, its complex definitions and specifications can be accommodated.

E-Marketing Management - An application specifically designed to automate your online and email marketing efforts. With this application, you can define and manage your campaign demographics, email message contents (text or html), email lists, email subscriptions, email templates, and email distribution schedule. After the completion of the email campaign, an online report showing click through and subscription counts will aid you to target and serve your market better.

Support Management - A workflow based application that provides full cycle customer support request management via the website. This application allows constant communication, collaboration, and status reporting between customers and companies providing the online support. This application allow for custom support form integration, request workflow and routing, access to historical and current status updates, and email notifications and confirmations.

Full Cycle Marketing Methodology

We patterned the Web Suite product after the marketing methodology we developed and implement for our customers.  This marketing methodology is called Website Full Cycle Marketing.  It is this methodology that makes our Web Suite product unique in the industry and in the market.  Below is a simple diagram that shows what the methodology is all about.

Web Suite Methodology

PUBLISH - At the center of the methodology is the company's website.  In order to do effective marketing, you need up to date contents.  If it is not easy to publish, change or update your website contents, then it lowers your website marketing effectiveness.  The Website Content Management application makes it easy for non-technical people to publish contents.

CATALOG - The company's website also need to address the products your company is trying to market.  Having a product catalog in your website is key to marketing your products.  The hardest part to having a product catalog is its maintainability and manageability.  The Product Management application makes it easy for any company to configure and build their product database and sell it effectively over the website.  This application can be easily integrated with any internet payment gateway and transaction processors.

MARKET - Now that a website with the proper information and product catalog has been established, it is now ready to be marketed and promoted.  The e-Marketing application addresses cold to hot email marketing.  This means the solution offers the flexibility to market to a fresh email list or to existing customer email database.  Email marketing can be ad hoc and automated based on a configurable email sending schedule.  This application is the one that produces the interest for the company's products and services.

SUPPORT - Marketing should be 24 X 7 operations by any company.  Once you have captured a sales via the website, the need to support and provide services to your customers is key to getting the next sale.  The Customer Support application is the communication bridge between your customers and your company and can be used as a means to learn what other products and services can be marketed to customers seeking support.  In our marketing methodology, customer support is considered and up sell mechanism.

FEEDBACK - It is important to keep your customers buying and happy.  In order to accomplish this, you will need to know how your customers feel about your company, products, and services.  This last step of the full cycle allows you to feel the pulse of your customer and based on the results of any anonymous or named surveys, you will be able to adjust and improve significantly.

It's expensive to manage website contents - If your company still utilize a highly paid programmer to make website content changes, then this solution is right for you. This solution enables regular non-technical users to update and publish contents.

Our website contents have not been updated for the longest time - Stale contents can damage the reputation of your company. Web Suite Integration Package (Existing Website)
If you have an existing website and you desire to integrate and utilize the Web Suite product to enhance the managability of your website, we offer the Web Suite Integration Package for existing websites for only $7,500.  This package includes the Web Suite product and integration of the Web Suite applications into one existing website.  The implementation time is only 1 week.  If you have more than 1 website, we can integrate the Web Suite application for $2,500 for each site.  The following services are provided in the 1 week implementation time period:

San Francisco, March 13, 2002 -- On Lok gains “organization of the year” honors in the Public Health Heros program for its contribution to promoting the health of older adults. The Public Health Hero program began in 1996 as an avenue for the School of Public Health at UC Berkeley to increase awareness of the health field.

The Public Health Hero program recognizes organizations and individuals for their significant contributions and exceptional commitment to promoting and protecting the health of the human population. A gala celebration will be held on Friday, March 15, 2002 at the Rotunda, 300 Frank Ogawa Plaza, Oakland.

On Lok has a long history as a non-profit health and community based long-term care program serving older adults who want to remain living in their own homes despite complex chronic illnesses. “For 30 years, our health program has been providing care, dignity and independence to older adults in San Francisco,” explains Jennie Chin Hansen, Executive Director. “Our members, whose average age is 82, receive complete medical, pharmaceutical, nursing, rehabilitative, recreational, and in-home care services as well as transportation and meals to help them live in their homes and communities.” On Lok’s program is the prototype that spawned PACE, the first new Medicare provider program since Hospice. Federal legislation signed into law in 1997 made PACE available in all 50 states. Now 26 PACE programs nationwide have replicated the On Lok model.

On Friday, May 3, 2002, On Lok celebrates its 30th Anniversary with a night filled with inspiring reflections in the midst of a turbulent health care system. On Lok Lifeways centers provide a welcoming, safe place for our members to receive outstanding healthcare services, as well as to socialize and make new friends. Each center gives seniors easy access to healthcare services within their nearby communities. All of our centers have a fully operating clinic, complete with exam and treatment rooms, and a rehabilitation gym designed to help our members regain and maintain functional independence.

In addition to the clinical facility, our day centers serve as pleasant meeting places, providing excellent opportunities for social interaction and engaging activities. We offer many group recreational activities, such as art classes, ethnic and cultural activities, and board/card games, as well as group outings to events such as trips to a Giants’ games or visits to local museums. We also offer a noon meal service, under the direction of our dietitians, that gives our members an opportunity to socialize and the nutrition they need to maintain a healthy diet.

The name On Lok means "place of peace and

happiness" in Cantonese, and this reflects both

our roots and our philosophy of care. Through five

related not-for-profit entities, On Lok provides healthcare, housing and intergenerational programs that meet the needs of seniors. The largest and

oldest component of On Lok’s services is On Lok Lifeways, whose mission is to provide quality, affordable care services for the well-being of the

frail elderly. We do this through On Lok Lifeways, a fully-integrated and

licensed health plan that delivers medical and long-term care to those looking for

an alternative to a nursing home. At present, On Lok Lifeways helps about 950 elder adults maintain their independence – by providing all necessary primary and specialty medical care, adult day health care, in-home health and personal care, social work services and hospital and nursing home care.

Serving the elderly for over 30 years, On Lok has

grown from its humble roots as a senior day

healthcare center to a complete medical and health

-related services program that is now replicated throughout the United States.

In the Beginning

1971

On Lok began as one of the country's first senior day health centers, a place that took care of older adults in the Chinatown, North Beach and Polk Gulch neighborhoods. Elders in these communities received hot meals, health and social services and supervision during the daytime, while returning to their homes in the evening. Our concern about the fragmented care we saw, motivated us to be innovative and start creating our own holistic senior healthcare plan.

1975

In-home support services, then primary care services, and case management of acute and chronic health services were added as On Lok provided services.

1979

On Lok launched a Medicare-funded demonstration of our consolidated model of long-term care through On Lok Senior Health Services. An interdisciplinary team (physicians, nurses, physical and occupational therapists, social workers, dietitians, health workers and drivers) formulated, coordinated care plans and provided all medical care and social services.

Demonstrated Success

Not only were our programs innovative and successful, but the cost of care was 15% less than the traditional fee-for-service care system. In 1983, On Lok obtained waivers from Medicare and Medicaid to test a new financing method for long-term care: a form of risk-based capitation. In exchange for fixed monthly payments from Medicare and Medicaid for each enrollee, On Lok was responsible for delivering the full range of healthcare services, including hospital and nursing home care – bearing full financial risk. A permanent Medicare and Medicaid waiver followed in 1986, and since that time On Lok has had no cost overruns and has been able to place a percentage of operating revenues in a risk reserve fund each year.

Nationwide Adoption

Also in 1986, On Lok’s ground-breaking model of coordinated service delivery and innovative financing began replication under the new name, the Program of All-inclusive Care for the Elderly (PACE). Today more than 70 organizations in 30 states are in various stages of the PACE model - from start-up to full operation. In the Balanced Budget Act of 1997, PACE became a permanent provider type under Medicare, and states gained the option of paying a capitation rate for PACE services under Medicaid. Thus, the undertaking that started with a few dedicated people in San Francisco has blossomed to become a nationwide movement whose continued goal is to create a better life for the frail elderly.

On Lok Lifeways is a comprehensive health plan

serving seniors who live in San Francisco, Fremont, Newark, Union City and Santa Clara County (not including Gilroy, Morgan Hill or San Martin). This program provides an important alternative when nursing home care and placement seem necessary. With On Lok Lifeways, you or your loved one can remain within the community, enjoying the comforts of home and family, for as long as possible¡­inclusive of medical and personal care assistance.

 

With the support of a comprehensive package of services, care is coordinated and delivered by a dedicated team of:

Each member¡'s needs are monitored and addressed. Based on member and caregiver input, the team puts together an individualized healthcare plan and then continuously assesses the member¡'s status. Benefits Plan
Summary of Benefit Plan

Medical Coverage

Employees and their eligible dependants can enroll the first day of the following month of your hire date.

You can choose to be covered under one of the following plans: Kaiser Permanente, Health Net or Chinese Community Health Plan.

 

On Lok pays your employee only premium for Kaiser and CCHP.  Your premium contributions are made with pre-tax dollars.

 

Dental Coverage

Eligible after 6 months of employment

You can choose to enroll in the Delta Dental DPO, which provides preventive, minor, major and orthodontia services. If a Delta Care dentist is selected, you will receive a greater level of benefits.

 

On Lok pays your employee only premium. Your premium contributions are made with pre-tax dollars

 

Basic Life & AD&D

Eligible after 6 months of employment.

On Lok provides life insurance policy of $10,000 at no cost to you.  In the event of your accidental death, your beneficiary will receive benefits equal to your basic life insurance benefit.  A percentage is payable for loss of limbs and sight.

 

Voluntary Life

Eligible after 6 months of employment

 

You may elect coverage up to 5 times your annual salary to a maximum of $300,000. This plan is at the expense of the employee.

 

Dependant Life

Eligible after 6 months of employment

You can elect coverage up to $50,000 for spouse, or up to  $10,000 for each dependent child without health questions. This plan is at the expense of the employee.

 

Personal Insurance

Eligible after 6 months of employment

On Lok offers a variety for Voluntary Benefits through AFLAC.  These benefits include a Personal Accident Plan with Short Term Disability, a Cancer Expense Protection Plan and a Volunteer Indemnity Plan.

 

These policies are at your expense, yours to keep and are completely portable and are available at group rates through payroll deduction.  Many of the premiums are based on age and level of coverage purchased.  The employee-paid premiums may be deducted on a pretax basis.

 

Flexible Spending Accounts

Eligible after 6 months of employment

 

You can enroll in one or both of the following flexible spending accounts (FSA), which allow you to pay for eligible non-reimbursed Medical/ Dental/Vision (max. $2,500/calendar year) and/or Dependent Care (max. $5,000/calendar year).

 

Tax Shelter Annuity Plan (TSA)

Eligible after one year of employment for On Lok matching contributions.

 

You may elect to enroll as of your hire date

 

On Lok will contribute 6% of your annual base salary if the employee contributes a minimum of 3%.

 

You are fully vested immediately.

Commuter Checks

Eligible on the first day of the following quarter of your hire date.

 

(Jan/April/July/Oct)

A benefit available to regular and limited status employees who use public transit, car and/or van pools for their commute.  You may deduct up to 100.00 per month from your salary on a per-tax basis to purchase commuter check vouchers.

 

Direct Deposit

Eligible on date of hire

Participation in direct deposit provides you the greatest safety and convenience, in addition to saving time and money for you and On Lok.  You can elect to have your paycheck automatically deposited into a checking or savings accounts.

 

Vacation

Vacation hours are earned based on the number of hours worked up to 40 hours in a workweek.  Regular full-time employees earn a maximum of:

13 – 24

25 – 96

97 +

 

Full-Time – Annual accrual

80 hours  (2 weeks)

120 hours (3 weeks)

160 hours (4 weeks)

200 hours (5 weeks)

Holidays

Full-Time - You are granted a total of 11 holidays each calendar year.

Part-Time -You are granted holiday hours on a prorated basis.

 

Sabbatical

A sabbatical of 280 hours will be granted after 10 years of continuous regular status employment. Sabbatical leave does not replace vacation earned during the year that the sabbatical is granted.  Part-time employees accrue sabbatical leave prorated according to the hours worked.

 

Sick Leave

For full-time employees, you will accrue eight (8) hours per month after 6 months of employment. Part-time employees accrue sick leave prorated according to the hours worked.

 

Beavement

You are eligible for paid time off as a result of death of an immediate family member.  An employee with regular or limited status will be paid for up to 3 days and is prorated part-time employees.

On Lok Lifeways is a certified PACE (Program of All-inclusive Care for the Elderly) Program.

 

The Program of All-inclusive Care for the Elderly (PACE) model is centered around the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

 

PACE serves individuals who are aged 55 or older, certified by their state to need nursing home care, able to live safely in the community at the time of enrollment, and live in a PACE service area. Although all PACE participants must be certified to need nursing home care to enroll in PACE, only about seven percent of PACE participants reside in a nursing home nationally. If a PACE enrollee does need nursing home care, the PACE program pays for it and continues to coordinate his or her care.

 

History of PACE- Modeled after On Lok Lifeways

 

The PACE model of care can be traced to the early 1970s, when the Chinatown-North Beach community of San Francisco saw the pressing needs of families whose elders had immigrated from Italy, China and the Philippines for long term care services. Dr. William L. Gee, a public health dentist, headed the committee that hired Marie-Louise Ansak in 1971 to investigate solutions. They, along with other community leaders, formed a nonprofit corporation, On Lok Senior Health Services, to create a community based system of care. On Lok is Cantonese for "peaceful, happy abode."

The 30th Street Senior Center Activities Program offers health and fitness activities, lifelong learning, arts & crafts, citizenship classes, performing arts & dance, group outings and more—legal consultation, translation services, a brain fitness exercise program, a computer lab and Internet access, a beauty shop and a large outdoor garden.

Seniors 60 years and older who need assistance in organizing and managing their long-term/short-term care can receive help from our bilingual (English/Spanish) Case Managers.  The goal of the program is to reach isolated or at-risk seniors and help them maintain their independence in their own home.  Some of the services that we provide include:

} Paratransit services

} Assistance with Medi-Cal and Medicare counseling

} Referrals to money management services, legal services
  or medical appointments

} Advocacy and assistance with solving a variety of problems

30th Street's Nutrition Program helps seniors maintain proper health with hot nutritious meals. All meals are prepared and packaged in our kitchen at 30th Street. Each year we serve over 93,300 meals at several dining sites in southeastern San Francisco and deliver about 60,000 meals to homebound seniors. 

 

Our registered dietitian plans all of our meals to ensure they are healthy and meet seniors' nutrition requirements. We also offer therapeutic meals to homebound seniors who have special dietary requirements. Below is a brief description of each of our nutrition services.

 

} Nutrition & Health Education

Our nutrition classes are offered in English and Spanish and provided by the Consumer Education, Health and Nutrition Department of City College of San Francisco.  With special focus on adults 60 years and older, topics include: "Controlling Your Cholesterol," "Fun with Food Labels," "Facts on Fiber," "Preventing Food Borne Illness" and more.

 

} Congregate Meals

Seniors come together for meals in the dining room on the first floor of the 30th Street Senior Center and at five other dining rooms throughout San Francisco.  For more information about our Congregate Meals Program, click here.

The 30th Street Congregate Meals Program provides over 400 hot, nutritious midday meals to six dining rooms throughout San Francisco.  We prepare all these meals in our 30th Street kitchen.

 

Our registered dietitian plans all meals to make sure they are low-fat, include lots of fruits and vegetables, and are healthy for seniors.  We make all meals from scratch and offer a variety of foods, including Latino, Asian and traditional American.

 

30th Street Dining Room

The 30th Street Dining Room is a popular lunchtime social setting.  Located on the first floor of the 30th Street Senior Center, the Dining Room offers a three-course, nutritious noon meal to seniors in the community.  Seniors have an opportunity to socialize and make new friends; that is especially beneficial to seniors who live alone.  We have one dining room coordinator on staff and many volunteers who help serve the meals.  Most volunteers are seniors from the 30th Street Senior Center.

 

The 30th Street Dining Room offers two meal seatings daily, at 12:00 PM and 1:00 PM, Monday – Friday, serving over 100 people at each seating.  We encourage a donation for each meal (suggested amount: $2.00).  Meal tickets can be obtained at the 30th Street Senior Center on the third floor.  For more information about our congregate meals program, call 415-550-2226.

30th Street Home Delivered Meals Program delivers hot, nutritious meals and special diets to homebound seniors unable to shop or prepare meals themselves.  We prepare and package all meals in our 30th Street kitchen. We offer therapeutic meals, such as low-fat, low-sodium and/or mechanical soft diets, for seniors who require special diets.  Nearly half are doctor-prescribed therapeutic meals. Also, we offer a culturally diverse menu, including Latino, Asian and traditional American foods, in order to appeal to the ethnically diverse seniors who live in the neighborhoods we serve.

 

We also offer nutrition counseling to home-bound seniors. Our registered dietitian can help seniors, who get only one meal a day from us, make healthy choices for their other meals.   

 

We prepare and deliver over 190 meals a day, making our Home Delivered Meal Program the second largest in San Francisco. We deliver hot meals Monday – Saturday and provide a frozen meal for Sunday. For more information about 30th Street Senior Center’s Home Delivered Meals, please call 415-550-2226.

 

On Lok's 30th Street Senior Services provides a spectrum of care for seniors in the Mission, Noe Valley, Diamond Heights, Glen Park and St. Mary's Park neighborhoods of San Francisco. Services offered by 30th Street Senior Services include:
